Query- Multiple Providers with <Ctrl>

When query appt, and adding multiple providers, rules listed like this select provider, or provider, or provider. When the dialog box is open, multiple providers are selectable while holding ctrl, BUT only first provider selected is added to query.

The gender field in F6 (New Patient screen) does not include the appropriate heading/ title.
